Sureway Health Support agrees to provide you with treatment at the price set out below. All prices are GST inclusive (if applicable) and include the cost of providing the treatment.

How to apply for a rebate
Medicare Rebates (under the Better Access to Mental Health Care Initiative)
- Book a double appointment with your GP to obtain a Mental Health Treatment Plan.
- During your appointment, ask your GP to refer you to Sureway Health Support.
- Provide a copy of your referral and Mental Health Treatment Plan to Sureway Health Support prior to your first appointment.
NDIS (National Disability Insurance Scheme)
The NDIS provides support to eligible people with intellectual, physical, sensory, cognitive and psychosocial disability.
